My uncle has agreed to meet up again so we can talk about his past. He said he felt better immediately after our last talk but then began to feel even worse later that night. He seems open to the idea that talking about it again might "relieve some of my tension".

Before we hung up, I asked him if he had any letters that he never wound up sending, that maybe he saved those along with the clothing. He laughed and said no, that was a team job. I asked him what he meant by a team, and he said that the puzzles were something that he and a friend would come up with together. He told me that a good friend of his also struggled with aggression, and they would compose the communications together. I asked him for the name of his friend and he said that they each promised one another they would never talk about their crimes with anybody else. I asked him again, just told him I was curious and wouldn’t ever tell anybody about his friend. He said it doesn’t really matter anyway because his friend died a long time ago. His name was Lee. I will record our next meeting.

Exactly. And I’d ask what the solution was to the 340 and how it was coded, just to nail it all down. Anyone alive who claims to be Z could easily prove it. "Confessions" short of that are almost certainly BS.

With mild to moderate Alzheimer’s disease, long-term memory stays relatively intact. But LTM suffers when the disease enters the severe range. Being delusional indicates his uncle has strange beliefs that have little relation to reality and persist despite evidence to the contrary. I’m not saying there is no way this man might be the Zodiac, but with his troubled mental health and deteriorating cognition, it would be very difficult to pin down his story based on what he is telling nephew. The only way to verify this man’s story is for the nephew to gain possession of the alleged physical evidence and take it to LE.
